Note: Not all Health Boards will go live with the updated SCI Gateway referral forms at the same time. Please look out for updates from your local Health Board primary care team on when the new forms will be available in your area.
Overview
Since 2013, five SCI Gateway referral forms have been used by the majority of Health Boards to refer patients from primary care optometry to the Hospital Eye Service (HES):
• Cataract
• Glaucoma
• Wet/Exudative AMD
• Paediatric
• General1
Following an extensive review, these referral forms have been updated by a national multidisciplinary group (representing primary care optometry and ophthalmology). The updated forms standardise the quality and consistency of referral data across Scotland, align with new ways of working and facilitate improved communication with the HES.
While some referrals include more data than previously, this reflects the information required by the HES to support effective triage and management. Every effort has been made to ensure the forms remain efficient to complete.
1 Some Health Boards have developed additional referral pathways, but the ‘General’ SCI Gateway referral form is often used for these.
