Glaucoma referrals
The glaucoma referral form is aligned with SIGN 144: Glaucoma referral and safe discharge, which sets out evidence-based recommendations and best-practice guidance for the assessment and referral of patients with suspected glaucoma.

Note: If referring a patient to the Hospital Eye Service who is currently registered under the Community Glaucoma Service this should be identified on the referral form using the relevant dropdown menu.
